GP Exercise Referral Scheme
The GP Exercise Referral scheme has been designed to help individuals self-manage certain conditions effectively and with confidence.
Our GP Exercise Referral instructor will find out what is important for you and help to find activities you enjoy!

Once referred by your GP you will be sent a questionnaire link which will need to be filled out before we can progress. You will also be sent further information about the programme.
Please note: You will need to be able to receive texts (either yourself or relevant carer/family member)

- Fibromyalgia or chronic pain

- Stress
- Muscle and joint pain
- Anxiety
- ME
- Hyperlipidaemia
- Hypertension
- Metabolic syndrome
- MS
- Osteoarthritis
- Osteoporosis
- Parkinson’s Disease
- Stroke
- Asthma
- Covid-19 rehabilitation
PLEASE NOTE THIS PROGRAMME IS NOT FREE
The programme runs as a rolling 6-month discounted membership which gives unlimited uses of the faculties along with all the GP Referral Classes and Activates.
(If you are already a member with Everyone Active, this will be converted for 6 months then revert)
Monthly Price: £32.99 per month we discount it by 25% to £24.74 for 6 months (comes out 1st of the month each month)
In addition, we also have Cardiac Rehab and Steady and Strong and Parkinsons rehab classes on a pay as you go basis at £5.00 per session at both Fareham and Holly Hill Leisure Centres. (Please contact the Leisure Centre directly for these classes)
- Steady and Strong classes can be attended to on a pay as you go basis and do not require a referral but can be used as part of the GP Referral membership if needed.
- Cardiac Rehab usually requires the patient have completed Phase III and be referred from that pathway.
- PD Warrior Parkinson’s Rehab classes are now running.
